    The Mother's Remedies Company Chicago Order Form Blank Paper Vintage Cure 8 1/2"
    This is an order form blank from "The Mother's Remedies Company" in Chicago, Illinois ( 1035 Thirty-Fifth St.) that measures 8 1/2" x 5 1/2". The form has text on both sides and is in folded state. No writing. Some age yellowing. This appears to be a mail order company that specialized in cures and sent products to sellers via the mail. This is an original item, not a copy.
